Every month our practice rewards those who help us to help other people. Education and communication are paramount to good dental health and well being. So by acknowledging the good work of our patients, we hope to better serve you. The rules for the Patient of the Month award are:
1.No purchase necessary.
2.Void where prohibited.
3.Patient of the Month is conducted in the first week of every month.
4.Selected patients agree to the below terms and conditions.
5.Selected patient must present within 30 days of telephone notification.
6.Entrants are all patients of the practice age 18 or older.
7.Selected patient must present within 30 days of telephone notification.
8.Selected patients are determined at random by drawing.
9.List of past selected patients can be obtained.
10.Odds of winning vary. Call us for details on this.
11.Winnings are non-transferable to another patient.
